Change format of invoice so applies to different users

I am a healthcare professional and use Mx Merchant for my patients' payments.  I would like the invoice not to read like it is only for product sales.  For example, instead of "Item" and a filing cabinet picture, a more general term that could apply to a patient name would help.  Or better yet-- let us customize and use the words that apply to our own services.
