Review Link and Survey Questions after Transaction completion

Can we have a 2-5 question survey after every transaction along with a review link for google, yelp, and social media?  The survey responses can be sent back every month as insights to the merchant.  The review postings and taking the survey can make the client a gift offer or generate a coupon for their next order as an incentive.
